Targeted Case Managers participate in the coordination of services and care with the following services and/or providers:

Primary Care Physicians
Department of Children and Families
Department of Juvenile Justice
Community Behavioral Health Agencies
Residential and Inpatient Treatment Facilities
​Outpatient Clinics
School Counselors
Natural and Community Supports
Assisted Living Facilities

Services We Provide

We assist program participants with identifying and gaining access to professional and community providers to address health, education, social, housing, vocational/employment, and daily living needs, as well as other needed resources.
